Service Description

This training will review the fundamentals of high-fidelity person-centered planning as well as the key components of the person-centered planning process, the role of assessments, how to assess and apply strengths to client identified goals, addressing a broad range of life domains, writing robust interventions, the role of the schedule card, and effective documentation.
